What is in it
A branded cover and how-to-run note. A baseline-to-interim change note with method prompts and Accept criteria. Tool adaptations with keep/drop/add criteria and hold/release. A report spine with five sections and pass/hold rules. An inception/interim outline with written criteria, hold/release, and owner checklist.
What you do
Fill the form. Read the preview (cover, change note, keep/drop/add, report spine, inception outline). Accept the mix. Get the zip. Put it on the shared drive.
What you walk away with

Who it is for
An evaluation manager at midline who needs a mid-cycle OS with written criteria, not a blank shell and not a second full ten-stage toolkit.
